Dr. Nadav Fields

Medical Director

Dr. Nadav Fields is the co-founder and medical director of CR8 Health. Dr. Fields received his bachelor’s degree from the University of Florida and completed his master’s degree in Biomedicine at Florida Atlantic University. He graduated from Touro College of Medicine New York and completed his residency training in Internal Medicine at the University of Arizona.

He is double board-certified by the American Board of Internal Medicine and the American Board of Obesity Medicine. Dr. Fields is trained in Bioidentical Hormone Replacement Therapy by the Preventative & Innovative Medicine Academy. Additionally, he is certified in Peptide Therapy and completed training with Dr. William Seeds, the world’s leader in Cellular and Peptide Therapy.

“Throughout his career, Dr. Nadav has worked in the emergency department, general medical ward, and in intensive care unit settings. It was here that he witnessed the sequelae of poor nutrition and lifestyle choices, which resulted in accelerated aging, obesity, heart disease, strokes, and premature death.

With growing frustrations in our healthcare system where we mostly focus on treating current disease states and alleviating symptoms, Dr. Fields became increasingly interested in Preventative Health with a focus on proactive not reactive medicine. This path eventually led him to opening CR8 Health where he can now engage, educate and empower his patient’s to reach their health destiny.”

Dr. Jonathan Fields was first drawn to studying Holistic and Eastern medicine over 20 years ago via his lifelong study of martial arts. After suffering for over a year with an undiagnosed and debilitating illness in his early thirties, and being completely let down by the conventional healthcare system, he was able to make a full recovery without any pharmaceuticals or surgeries. Being grateful for his second chance, and now having the ability to empathize with patients in dire need, he dedicated the rest of his life to empowering others with similar stories. Dr. Jonathan is also a published author, keynote speaker, tai chi, qi gong & meditation instructor, and transformational retreat host. He owns multiple integrative practices in South Florida, lectures at Universities & Fortune 500 companies, works with elite athletes & celebrities, and has formulated for publicly traded companies.

Liz Terrizzi APRN is a board-certified family nurse practitioner with specialty experience in Primary Care and a passion for Root-Cause Medicine. With over 20 years in the healthcare industry and trained in CR8’s proven system, Liz is driven to help patients achieve optimal health with personalized care plans that restore the body’s systemic balance, boost energy and enhance overall well-being. A proud Venezuelan American, Gabriela brings cultural sensitivity and bilingual support to her diverse patient base, offering care in both English and Spanish.

Educated in West Palm Beach, Florida, Gabriela’s passion for cutting-edge health solutions was ignited in New York, where she trained alongside experienced leaders in peptide therapy and integrative wellness. Since then, she has worked hand-in-hand with respected physicians, NP’s, and PAs across Florida, gaining in-depth expertise in hormone health, performance medicine, and personalized healing protocols.
